Neurolytic Blocks for Cancer-Related Pain

What is a neurolytic block?

A neurolytic block, also known as neurolysis or chemical neurolysis, is a procedure used to alleviate severe cancer-related pain.

The procedure involves injecting a chemical into the nerve which destroys the nerve or nerve plexus, and then disrupts the transmission of pain signals along specific nerves.

A neurolytic block is a complementary, novel procedure that reduces the need for opioid medications and improves the quality of life significantly for the patient.

When are neurolytic blocks used?

Neurolytic blocks are particularly effective in managing cancer-related pain that starts in specific nerve pathways. The procedure can be applied to nerves in various areas of the body, including the abdominal region (e.g., celiac plexus block), the chest (e.g., thoracic paravertebral block), and the pelvis (e.g., hypogastric plexus block).

Neurolytic blocks are offered to cancer patients who are experiencing severe localized or regional pain which is significantly impacting their quality of life.

These patients may have advanced-stage cancers or metastases that cause chronic, uncontrolled pain. Candidates for this procedure have typically tried conventional pain management methods, such as oral medications and radiation therapy, without achieving adequate pain relief.

What does the procedure involve?

Patients receiving neurolytic blocks will be seen in an outpatient clinic. They will be required to fast for at least 6 hours prior to the procedure.

At the time of the procedure, a sedative will be given. Using fluoroscopy or ultrasound, the target nerve is identified, and a needle administers the block.

After the procedure, the patient will be briefly monitored during recovery and will be able to go home the same day. A follow-up appointment will be scheduled at the clinic in 2-4 weeks.
