Request Your Medical Records

Accessing your medical records, Easily request, view, and manage your medical records through our secure Patient Portal app or website—no forms needed.

You can obtain a copy of your medical records from the Health Information Management Services (HIMS) Department. This can be done via Cleveland Clinic Abu Dhabi’s secure Patient Portal mobile app or website. There is no need to sign an authorization form beforehand.

Once requested, you will receive your medical records within 2 business days. You can download, save, and print your medical records as needed.

How to access medical records within the app

These steps guide you through submitting a medical records request using the Cleveland Clinic Abu Dhabi Patient Portal app. You start by accessing the request section in the app, then select the type of records you need, provide relevant visit or discharge dates, and explain the purpose of your request. This process helps ensure your medical documents are accurately prepared and delivered for your specific needs.

• Open the Cleveland Clinic Abu Dhabi Patient Portal App.
• Click Main Menu → click My Record → click Medical Records Request.
• Read the information carefully and choose the type of request → Add visit or discharge date.
• For Medical Report/Letter Request, explain your request and add the name of your physician and the purpose of the request (e.g. for insurance, IPC, personal use etc.).

 

 

Please Note

Imaging study CDs can only be requested in-person by visiting the HIMS Department (see below). There is a fee of AED 26.25 per CD.

1. Procedures are not provided on CD, but they can be provided as a report with all details included.
2. A written authorization, signed by the patient, must be obtained before requesting medical records on behalf of someone else.

You can request medical records in-person at the HIMS Department, located on level P1. Opening hours are 9:00am to 4:00pm, Monday to Friday. Please note, prayer time is observed on Fridays and the office is closed between 12:30 pm to 2:00 pm.
